the slope-intercept form is given by y=mx +b

Since the slope is 5, we can write:

y=5x +b

Pluggin in (5,4)

4=5(5)+b

4=25+b

4-25=25-25+b

21=b

b= 21

3) Finally, the slope-intercept form of this linear equation is y=5x +21
